The film's plot is secondary to its main attraction: the unflinching and unapologetic display of gore and bodily mutilation. The human centipede itself is a gruesome creation, with Lindsay, Jenny, and a third victim, Koji (Akihiro Kitamura), being stitched together in a nightmarish configuration.

The performances in the film are largely forgettable, with the exception of Dieter Laser, who brings a manic and unsettling energy to his portrayal of Heiter.
